Hi,
It happened to me, I solved this by:
# crm node delete node-with-old-hostname
Simple as that, after that the [offline] node left.
OS: CentOS release 6.3 fully upgraded with the following packages:
# rpm -qa | egrep 'pacemaker|corosync'
corosync-1.4.1-7.el6.x86_64
corosynclib-1.4.1-7.el6.x86_64
pacemaker-cli-1.1.7-6.el6.x86_64
pacemaker-1.1.7-6.el6.x86_64
pacemaker-libs-1.1.7-6.el6.x86_64
pacemaker-cluster-libs-1.1.7-6.el6.x86_64
Cheers!
